Chronic Disease Prevention
CDH supports lifestyle changes that lower the risk of chronic illnesses like diabetes, heart disease, and cancer. Through education, community partnerships, and evidence-based programs, we empower individuals to make healthy choices and build a healthier future.
Community Health Screenings
Regular health screenings are an important step in preventing chronic conditions and catching potential issues early. Knowing your numbers, such as cholesterol, blood glucose, and A1C, can help you make informed decisions about your health and reduce your risk for serious complications.
Our screenings are quick, simple, and informative:
- Single finger poke for a full lipid panel (Total Cholesterol, LDL, HDL, triglycerides, and blood glucose)
- A1C test for diabetes risk
- It takes about 20 minutes
- Participants leave with a copy of their results
