Disease Reporting for Healthcare Providers
Healthcare providers are required to report certain diseases and conditions to local health departments so that steps can be taken to prevent the spread of disease and stop others from becoming ill. These steps might include investigating risk factors and contact tracing, messaging to raise public awareness, removing a contaminated item from stores, or offering preventive treatments to people who have been exposed. All these interventions help us protect the public’s health.
Refer to the list of reportable diseases for information about the diseases that are reportable and the time frame in which they must be reported. Link: Download Reportable Disease Poster.
You can call 208-327-8625 or FAX 208-327-7100 and provide the disease being reported, date of collection and result date, patient name, patient DOB, patient address, and patient phone number. Or you can fill out an online CDH Communicable Reportable Disease Form.
