Support Programs

Community Health Workers

Our Community Health Worker (CHW) program was established to encourage healthy behaviors and help people to access essential services and community members.

Our CHWs serve as representatives of the people they assist and act as vital links between health and social services. Drawing on their personal experiences, CHWs collaborate with patients to assess social needs, home environments, and various risk factors to connect individuals with appropriate community-based resources.

Contact Us

Please call 208-954-3889 for partnership inquiries or to invite CDH to participate in an event.

Find Help Idaho

Visit findhelpidaho.org to access free and reduced-cost health resources and social services both within our community and across Idaho.

Community Closet

Our Community Closet is located in our Boise Clinic office in the lobby.

Through generous donations from the community, we are able to provide free clothing, shoes, and toiletries for our patients and community.

If you would like to donate, please drop off cleaned and gently used clothing at the Community Resource Center or call 208-860-5482.
