Vaccine Information for Providers

CDC – Vaccine Information Statements (VIS)

VIS’s are information sheets produced by the Centers for Disease Control and Prevention (CDC) that explain both the benefits and risks of vaccines to vaccine recipients. Federal law require that healthcare staff provide a VIS to a patient, parent, or legal representative before each dose of certain vaccines.

For Providers: Idaho’s Immunization Reminder Information System (IRIS)

IRIS is the free, secure and confidential statewide computer-based system that keeps track of immunization records to help providers and parents keep children up-to-date on their immunizations.

The Shot Line | 208-321-BABY (2229)

A phone service that provides answers and resources to those who are responsible for the administration and delivery of immunizations. The Shot Line is staffed during regular clinic hours, Monday through Friday.
