We accept Medicaid and most insurance plans and offer a sliding fee scale based on family size and income.
To determine if you are eligible for the sliding scale, please bring income verification for your appointment. Your income information needs to be your most current income statements and can include ONE of the following:
- Paystubs (two if paid bi-weekly, four if paid weekly)
- Prior year tax return
- Other forms of Proof of Income
Registration Forms
You must have a valid, accessible email account. Once you submit your completed forms, you will receive a message to the email account you shared. You will need to open this email and follow the instructions to complete the form submittal process. If you do not complete this step, CDH will not receive your forms.
Forms are mobile-friendly and can also be filled out from a desktop computer.
Registration is not required for the WIC program.

Savannah Klinginsmith, MSN, FNP-BC, CNL, joined Central District Health in 2019 after completing the Family Nurse Practitioner Program at Clarkson University in Omaha, Nebraska. Prior to that, she completed her Master’s in Nursing, specializing in Clinical Nurse Leadership, at the University of San Francisco.
“I wanted to advance my nursing career, emphasizing quality improvement with a hands-on, clinical approach,” Savannah says. “My goal has always been to work as a nurse practitioner and learning leadership skills and quality improvement with a clinical emphasis was a great stepping stone in preparation for my NP practice.”
Savannah has been a nurse since 2001 with experience in medical/surgical nursing, urgent care, family practice, gynecology, and occ medical. She says, “The opportunity to help grow CDH’s clinic and provide family care to many in our communities who may not have access to care is what brought me here.”
In her free time, Savannah enjoys spending time with her husband, two children, and their dog. She also likes running, hiking, reading, playing the piano, and exploring the great Idaho outdoors.

Natasha King-Anderson is a Family Nurse Practitioner who has always been interested in healthcare and helping others. She started working in healthcare as a nursing assistant when she was 17, then went to school to become a nurse and eventually a nurse practitioner.
Natasha has worked in pediatrics, urgent care, dermatology, and rehab. She says of working at Central District Health, “At CDH I get the opportunity to ensure our community has access to care, including preventive health, health screenings, and care management of acute and chronic conditions, regardless of one’s income or social status.”
Natasha attended Boise State University, College of Southern Idaho, and Northwest Nazarene University. In her spare time, she enjoys hanging out with her family, playing sports, camping, and traveling.
Andrea Gilman

Andrea Gilman, LCSW, EMDR-T, joined Central District Health as the Counseling Services Clinician with Family and Clinic Services.
Andrea, a licensed Clinical Social Worker, has a bachelor’s degree in criminal justice and master’s in social work from Boise State University.
She has worked for multiple non-profits in the Treasure Valley, as well as for the Department of Corrections, Department of Health and Welfare, and the Veterans Administration.
“My approach to counseling services and behavioral health emphasizes the mutual development of goals for therapy, along with an ongoing evaluation of the therapeutic relationship,” she said.
Andrea utilizes mindfulness, cognitive behavioral therapy, solution focused therapy, play/interactive therapy, polyvagal therapy, and eye-movement desensitization reprocessing therapy (EMDR-T), and is invested in an integrative, dynamic, and holistic approach to mental health.
When not at work she enjoys spending time with family & friends, anything sci-fi, baking, traveling, and lounging around on a Sunday afternoon. She listens to a variety of music and counts several television shows and films as her “guilty pleasures.
