Off-site Mobile Vaccine Clinics
We are committed to safeguarding the health of all members of our community, including RIM (Refugees, Immigrants, and Migrants), Latinx, and Rural communities, by providing convenient and free community off-site mobile vaccine clinics in a diverse and equitable environment. These clinics aim to build trust and promote optimal community health by eliminating transportation and healthcare barriers within the community.
Health insurance is not required for our services offsite. To ensure a seamless process, we may ask for historical medical records or knowledge of them prior to filling out our form.
We are currently accepting any requests to host or partner with agencies or businesses within our counties (Ada, Boise, Elmore, Valley) and will do our best to provide services off-site. Please submit a mobile clinic request here.

Immunization Records
Maintaining accurate immunization records is crucial for the success of our vaccine clinics. In all instances, Central District Health will refer to your immunization records before the clinic to ensure your vaccine records are correct. Our vaccine recommendations adhere to the guidelines set by the Center for Disease Control and Prevention’s (CDC) Immunization Schedule.
IRIS – Immunization Reminder Information System
If you have previously received vaccines in the State of Idaho, they will be reported to the above database, and we will have access to those records.
However, if you received vaccines in a different state or country if you have chosen to opt out of the IRIS registry, we kindly request that you bring your records with you to the off-site clinic.
Should we require any additional information or clarification regarding you vaccination record, we will promptly contact you via phone or email.
How to Participate
To ensure a smooth vaccination process, please bring any medical records as well as health insurance if you have it. Health insurance is not required but we will ask if you have any. Our forms take 5 minutes to complete and can be accessed at the start of our offsite clinic. These forms must be completed by the participant and serve as written consent for CDH to administer vaccines to you.
Offsite Mobile Clinic Day
To ensure a safe and efficient vaccination process, please consider the following guidelines:
- Make sure to eat something and drink water the day of vaccination.
- Dress in loose-fitting clothing or layers that allows easy access to the upper arm for the vaccine administration.
- Possible side effect of a vaccine includes soreness at the site of the injection.
