Women, Infants, and Children (WIC) Program
WIC is a supplemental nutrition program for Women, Infants and Children up to age 5. It provides nutritious supplemental foods, breastfeeding support, as well as nutrition education and referral to health care services at no cost to eligible families.

Use this form to submit your WIC application with Central District Health. Once submitted, expect to be contacted within 2 business days to complete your application. If you have any questions, please reach out to our CDH WIC office at 208-327-7400.
How to apply
- Submit the CDH online application below or call 208-327-7400 to apply over the phone.
- Wait up to 2 business days to be contacted via phone to schedule an appointment.
- Gather required documents. An option to attach these files in advance of your appointment is in the form below.
- Proof of physical address. (No P.O. Boxes)
- Proof of income for the last 30 days unless you are on Medicaid, CHIP, TANF or SNAP.
- Come in for your appointment with Proof of ID for all applying including adult applying on behalf of child/infant, and bring your required documents (if not already attached below).
