School Vaccine Clinics
We are committed to safeguarding the health of our community, especially our students, by providing convenient and free school-based vaccine clinics. These clinics aim to promote optimal health among children and minimize the number of sick days they experience.
Parental or guardian permission is a requirement for your student to receive vaccines at a CDH school-based vaccine clinic. To ensure a seamless process, we request that you complete the form below if you would like your student to participate in an upcoming school-based vaccine clinic.
We offer two different types of immunization clinics in schools:
- School-Required Immunization Clinics – During these clinics, we will provide your student with the immunizations they need for school!
- School Flu Vaccine Clinics – During these clinics, we are only providing the flu shot for your student!
For Parents
To ensure a smooth vaccination process, please submit the necessary forms at least 48 hours before the scheduled vaccination clinic. These forms must be completed by the student’s parent or guardian and serve as written consent for CDH to administer vaccines to your student.
We have provided a Fillable Form version for your convenience. This version allows you to complete and submit the form directly from your computer or mobile device, automatically sending it to CDH upon signature.
You need to complete one or both forms depending on what your school is requesting.
Maintaining accurate immunization records is crucial for the success of our vaccine clinics. In all instances, Central District Health will refer to your students’ immunization records before the clinic to identify the recommended vaccines for them. Our vaccine recommendations adhere to the guidelines set by the Center for Disease Control and Prevention’s (CDC) Immunization Schedule.
If your student has previously received vaccines in the State of Idaho, they will be reported to the State’s Immunization Reminder Information System (IRIS), and we will have access to those records.
If your student has received vaccines in a different state or if you have chosen to opt out of the IRIS registry, we request that you email your student’s vaccine records to us at our HIPAA secure email address: offsiteclinics@cdh.idaho.gov. This will enable us to provide the most comprehensive care for your child.
Should we require any additional information or clarification regarding your child’s vaccination record, we will promptly contact you via phone or email.
Preparing for Vaccination Day
To ensure a safe and efficient vaccination process, please consider the following guidelines:
- Make sure your child has eaten a healthy breakfast on the day of vaccination.
- Dress your child in loose-fitting clothing or layers that allows easy access to the upper arm for the vaccine administration.

Central District Health offers a Fall Flu Vaccination Program for schools within Ada, Boise, Elmore, and Valley counties. As part of our commitment to ensuring a healthy and safe learning environment, we visit different schools to administer flu vaccines to students. Vaccination is a crucial step in preventing the spread of influenza and reducing absenteeism due to flu-related illnesses.
Our dedicated team of healthcare professionals visits schools to administer flu vaccines to students. All students who participate in the program receive vaccinations free of charge. We encourage parents and guardians to take advantage of this opportunity to protect their children against the flu.
