VALLEY COUNTY
Monday through Friday 8 a.m. – 5 p.m.
ELMORE COUNTY
Monday through Friday 8 a.m. – 5 p.m.
Telehealth appointments are available for scheduling from 8 a.m. to 5 p.m. Patients must be in Idaho at the time of the appointment.
FORMS
IMPORTANT:
You must have a valid email account which you can access. Once you submit your completed forms, you will receive a message to the email account you shared. You will need to open this email and follow the step to complete the form submittal process. If you do not complete this step, CDH will not receive your forms.
The forms are mobile-friendly and can also be filled out from a desktop computer.
Your privacy is important to the Central District Health. We follow the regulations of HIPAA—the Health Insurance Portability and Accountability Act. You will be asked if you have read and understand the documents you submit.
Note: Central District Health is only able to provide health records, including for immunizations, of clients who have been seen in our clinic. If services were provided at a different clinic, please contact that clinic to request your health records.
RESOURCES
Sliding Fee Scale:
CDH offers a sliding fee scale based on family size and income. To determine if you are eligible for the sliding scale, please bring income verification for your appointment. Your income information needs to be your most current income statements and can include ONE of the following:
- Paystubs (two if paid bi-weekly, four if paid weekly)
- Prior year tax return
- Other forms of Proof of Income

COVID-19 Safety
In an effort to protect the health of our patients, staff, and community, any individuals over the age of two who have suspected or confirmed COVID-19 infection or who have symptoms including fever, sore throat, runny nose, cough, or nausea are required to wear a face mask in our clinics.
Individuals may also choose to continue masking based on personal preference and their perceived level of risk for infection.
